Welcome to the University of Pennsylvania Assessement of Daily Function Questionnaire

We would like your help with a questionnaire about how Parkinson’s disease affects daily activities. This survey is part of a study taking place at the University of Pennsylvania to learn about how Parkinson’s disease affects people’s lives. 

The daily function questionnaire has 98 items and should only take about 15 to 20 minutes to complete.  As a thank you for your time and efforts we will enter your name into a drawing for a chance to win $100.
